Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. SCHS understands the influence of the Social Determinants of Health upon health inequities experienced by community members and work with consumers and community members, to structure their care around the context in which they live and work.
This is an exciting opportunity for you to join an experienced, supportive, friendly, and dynamic dietetics’ team, and work in an organisation that has a broad range of multidisciplinary services.
As part of the team, you will:
- Provide high quality Dietetic services to clients from various funding sources g., CHSP, HACC, CH, DVA, NDIS, HCP.
- Provide relevant assessment, intervention, lifestyle coaching and clinical management of dietary related conditions and health behaviours.
- Provide Dietetic interventions that support the health, wellbeing, and self-management of all clients.
- Complete documentation and collect appropriate data to meet relevant billing and reporting requirements, and to ensure compliance with privacy legislation.
- Ensure that care services are accessible, culturally relevant, and inclusive.
- Be working with clients that have complex comorbidities, psychosocial issues, and financial difficulties that all impact on their nutritional status.
