CP@team
CP@team is a chronic disease prevention, management, and health promotion service. The aim of the CP@team is to support members of the community manage their health outside of the emergency setting. The CP@team aims to:
- Reduce calls to emergency services
- Improve clients quality of life through self-care, ability to engage in usual activities
- Reduce chronic disease risk
- Empower clients to take control of their health and set goals based on their chronic disease risk factors
- Connect clients to primary healthcare servcies where possible
What happens at a CP@team session
Paramedics conduct 1-on-1 sessions with clients on a regular basis (weekly, biweekly, or monthly)
New clients can drop-in at any time within the designated time slot during the sessions the community paramedic may: - Conduct several evidence-based health assessments
- Provide referrals to primary care and community resources
- Assist clients in navigating the health care system
- Provide clients with tailored health education
- Engage participants in healthy lifestyle discussions
- Set health goals with participants
Community paramedics with program specific training are available at the CP@team to provide members of the community with regular blood pressure checks and health asessments in the area of chronic conditions. Paramedics take a holistic approach to all new clients and offer clients advice and provide support and assistance to faciklitating referrals to health and social services.
The aim of the CP@team is to provide members of the community that are at risk or are diagnosed with chronic health conditions and reduce the number of emergency service call outs and visits to the emergency department for primary health care needs.
The CP@team model has been developed by McMaster University (Canada).
