What does it involve?
SCHS HomeBase Services are provided to young people who live and/or work in the Sunraysia Region including; Mildura, Merbein, Irymple and Red Cliffs. HomeBase works within a Social Determinants of Health Framework to meet the health needs of young people, no matter how straight forward or complex.
Service Information
The HomeBase health clinician provides youth focused interventions and holistic ongoing case management to youths 10-25 years of age and their families to improve their health, wellbeing and social connectedness.
The HomeBase health clinician aims to bridge support gaps with youth through collaboration with services, family and can assist with appropriate connections and referrals to internal and external support services as well as meaningful activities and assist with gaining basic needs.
Eligibility
Who can use this service?
Young people aged 10-25 years. HomeBase Health recognises the significance of families and informal social support networks and, with client consent, includes these people in their interactions with the young person.

This is a free service.
