Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. The Role
We are looking for motivated Registered Nurses with highly developed clinical practice, decision making, client advocacy and sound assessment skills to join the District Nursing team in a casual capacity. We are a fast paced leading provider of health care in the community who service a wide variety of clients that includes general nursing, chronic and complex care and palliative care. As a member of the District Nursing team:
- You will be responsible for providing quality nursing care for a diverse group of clients
- You will use a problem solving approach involving assessment, planning, implementation and evaluation in the provision of quality nursing care to the clients
- You will provide health education to clients and their carers about their identified health needs by creating an environment which enables them to be responsible for their own health
- You will liaise closely with community service providers to coordinate client care
- You will act as client advocate to assist positive health outcomes
