Social & Community Support Program
Our Service
Social & Community Support program is a community care program providing services in the community to support people under the age of 65 years, or 50 years for Aboriginal and Torres Strait Islander Peoples, who have difficulty with daily activities assisting recipients to be more independent and to participate in their community.
How it works
- We’ll check your eligibility. We may be able to do this at intake, or we may need to refer you for a holistic living at home assessment, depending on your needs.
- We’ll work with you to develop a plan
- We’ll help you develop new skills, build your capacity & grow your independence
The aim of Social & Community Support is to:
- Provide community care services, both in the home and in the community, to
- Support people who have difficulty with the activities of daily living.
- Support people to improve their health, wellbeing and safety.
- Provide a coordinated and integrated range of supports to assist people to remain as independent as possible.
- Provide flexible, innovative, contemporary, timely and responsive services to support people to engage and connect in their communities.
Who can access this service?
- Social & Community Support program generally supports people who are not eligible for the NDIS or have not yet tested their eligibility for NDIS.
- People from birth to age 65, or 50 years for Aboriginal and Torres Strait Islander peoples with chronic illness, mental health, disability, and their unpaid carers’.
- Eligibility means that the person is assessed as being in the target group under HACC-PYP funding.
Eligibility for services is not based solely on age, but on the level of difficulty a person experiences in carrying out tasks of daily living. - There is no restriction on access to HACC-PYP services based on residency status or visa type.
