Seniors Lifestyle Victoria Program
Eligibility
You may be eligible if you:
Are 65 years or older, or 50 years or older for Aboriginal or Torres Strait Islander people
- Live in the community (not in residential aged care)
- Are experiencing changes to your health that affect your daily life, including;
- Reduced mobility, strength, balance or endurance
- Unintentional weight loss
- A fall or unplanned hospital visit in the past 12 months
- Are experiencing social isolation or loneliness
Cost
Participation in the Seniors Lifestyle Victoria Program is free.

Seniors Lifestyle Victoria Program
Seniors Lifestyle Victoria offers support for people who are living in the community and may benefit from support to stay strong, active and connected.
The program aims to:
- Improve the health and wellbeing of older Australians
- Help people remain independent in their homes for longer
- Reduce avoidable hospital visits and admissions
What the program provides
Through Seniors Lifestyle Victoria, you can access a range of local services to support your health and wellbeing. This might include help with physical activity, nutrition, medication management and social connection.
You will work with a dedicated coordinator who will get to know you, your health, and what matters most to you. Together, you will develop a personalised plan tailored to your individual needs and goals.
