To be eligible for this service you should be:

  • Aged over 18 years
  • Experiencing persistent pain  (three months or more)
  • Looking for ideas about how to better manage your pain
  • Not currently under workcover or TAC

For people with chronic or persistent pain, the SCHS pain rehabilitation service offers a comprehensive assessment of your pain issues, including recommendations to your GP and a wide range of treatment options.

What does a pain assessment involve?

Pain is complex and usually has multiple causes and contributing factors. For many people these factors are not well understood. By undertaking a full pain assessment with several members of our multidisciplinary team, you can gain a clear understanding of the factors contributing to your pain and, with the help of our team, put together a treatment plan to manage it more effectively.
The treatment plan and recommendations are sent to you, your GP, and any other health providers you nominate. This ensures that everyone involved in your care is on the same page.

What then?

Where there is persistent pain, most people find that using just one treatment strategy, such as medication alone, is ineffective. Taking a ‘whole person’ approach to pain means addressing all your pain contributors. This can include how you move, your daily activity, fitness, sleep, stress management and general health. All these things can be effective in managing pain and improving quality of life and can be included in your treatment plan. Because pain is a variable experience, treatment is tailored to the individual. Treatments can include exercise (group or 1-1); mental health support; medication adjustment; health coaching; help with activities, sleep, and daily routines; and referral to other medical, health or social support services. The aim is for you to return to the care of your GP with a better understanding of your pain and more skills to manage it yourself.

How to access the pain rehabilitation service?

Referred by a healthcare provider (GP, physiotherapist, practice nurse etc.) or self-referrals accepted.

You will be contacted by our staff and booked in to attend one of our 90-minute ‘understanding pain’ sessions which explain what pain is and what treatment options are available through our service.

On completion of the ‘understanding pain’ session, you will be provided with a pain rehabilitation-specific questionnaire. Once this is returned you will be booked into our intake program with the pain rehabilitation coordinator.
