Eligibility

It is important to see a Diabetes Educator when:

  • Diagnosed with diabetes— Type 1, Type 2
  • Having difficulties reaching treatment targets and management goals including blood glucose, weight, blood pressure and cholesterol levels
  • Pre-pregnancy planning, during pregnancy and post-delivery care.
  • Change in diabetes management such as changing tablets, commencing insulin or other injectable therapy
  • Life stage changes such as starting a new school, puberty, adolescence, or menopause
  • Feeling stressed or ‘burnt out’ by diabetes
  • Planning to travel extensively

Cost

There may be a cost for this service based on your income level. Seeing a Diabetes Educator is an opportunity to receive updated education, information and support.

Service Information

Our team of qualified Diabetes Educators will work with you, your Doctor and Diabetes Team, to assist you to achieve the best health outcomes. We can initiate referrals to other health professionals to meet your individual needs. We have free access to specialist services at Baker IDI Endocrinology via Telehealth.

Have You Seen A Diabetes Educator In The Past 12 Months?

Diabetes is a complex disease where the regulation of blood glucose (sugar) is altered. This can affect the entire body, in particular; the eyes, feet, kidneys, heart and brain. It is the fastest growing chronic disease, with around 280 Australians diagnosed every day.

While the treatments for diabetes might vary, one thing stays the same – you play a big part in managing your diabetes and improving your own health.

Research indicates that people who understand their own condition, are better self-managers, resulting in improved health, wellbeing, and quality of life. Diabetes related complications can be prevented or delayed. Best practice recommends people with diabetes have an annual review to monitor their diabetes management and progression of the disease. More frequent appointments are determined by you and the diabetes educator depending on your individual needs.

How Can It Help Clients?

Diabetes Educators support people with diabetes, their families and carers to gain the knowledge, skills and confidence they need to actively manage their condition.

Diabetes Educators assist people with diabetes to:

  • Know what to do
  • Know how to do it
  • Want to do it
  • Be able to do it
  • Recognise when to seek assistance

Health coaching and motivational interviewing techniques are used to assist people to make and sustain behavioural change.
