This program aims to empower people living with chronic disease(s) to better manage their health, to improve their quality of life, and reduce avoidable trips to hospital.

Our team of qualified Diabetes Educators will work with you, your Doctor and Diabetes Team, to assist you to achieve the best health outcomes. e can initiate referrals to other health professionals to meet your individual needs.
Dietetics
Our Dietitians provide tailored and expert nutrition advice that works for you. By listening to your needs and working with you, they ensure that an appropriate treatment plan is prescribed, with achievable, personalised goals.
Podiatry
Our Podiatrists can help you by listening to your needs and working with you to plan the most appropriate treatment for you, including setting goals and developing a treatment plan. You will be actively involved in your podiatry care.
