The Role

Seniors Lifestyle Victoria is focused on improving functional capacity, enhancing wellbeing, and promoting independence among older Victorians experiencing mild to moderate frailty. The program aims to reduce, slow, or potentially reverse the progression of frailty, thereby helping to minimise avoidable emergency department presentations and hospital admissions.

The program delivers coordinated, person-centred supports that include physical activity, nutrition, medication support, social connection, and health coaching. These supports are tailored to individual goals and strengths, and responsive to local community needs, to promote sustained improvements in health, independence, and quality of life.

The Exercise Physiologist is responsible for delivering Exercise Physiology services within the Seniors Lifestyle Victoria program, including individual assessments and consultations, the development of tailored exercise interventions, and the facilitation of group-based programs.

The role may also include Care Coordination responsibilities, working collaboratively with participants, their families, carers, and multidisciplinary teams to develop, implement, and monitor personalised action plans aligned with participants’ goals and needs. Through a person-centred approach, the Exercise Physiologist supports improvements in health, wellbeing, functional capacity, and independence, while facilitating access to health, aged care, and community services, supporting navigation of complex systems, and promoting effective communication between providers to ensure coordinated, holistic care.

Key Selection Criteria

Mandatory

  • Tertiary qualifications in Exercise Physiology recognised by Exercise & Sports Science Australia (ESSA).
  • Current accreditation as a practising member of Exercise & Sports Science Australia (ESSA).
  • Current driver’s licence.
  • Highly developed verbal, written and interpersonal communication skills.
  • Proficient information technology skills, including experience using digital client management systems.

Preferred

  • Demonstrated experience in supporting self-management using coaching and/or motivational interviewing approaches for individuals with conditions that impact functional capacity.
  • Strong understanding of the Social Determinants of Health and application of person-centred, strengths-based practice principles.
  • Proven ability to work effectively and collaboratively within multidisciplinary teams and across diverse communities.
  • Knowledge of, or experience working within, My Aged Care, Support at Home, and other relevant community-based aged care programs.
