Sunraysia Community Health Service (SCHS) is a dynamic organisation promoting health, wellbeing and independence in a social model of health. The organisation is a source of primary and preventive care for our community members who are at risk of poor health outcomes, and experience acute health disparities. SCHS understands the influence of the Social Determinants of Health upon health inequities experienced by community members and work with consumers and community members, to structure their care around the context in which they live and work.
The Role
This position will support Commonwealth Home Support Programme (CHSP) service providers through reforms to the CHSP, in preparation for a new in-home aged care system and to operate effectively in line with the objectives of the CHSP and within the context of the broader aged care system.
Applicant Requirements:
- Demonstrated knowledge and understanding of the CHSP reforms, from a conceptual to an operational and practice level.
- Demonstrated knowledge and experience in the community care sector and related service systems in Victoria.
- Tertiary qualification in a relevant discipline.
The successful incumbent will be required to undertake a national police check.
