First Nations People living in the Loddon Mallee Region, who have been diagnosed with a chronic illness.

SCHS AHPACC

Jock Peterson is the SCHS Aboriginal Health Promotion and Chronic Care worker at SCHS and works closely with the community, providing support services and encouraging preventative health care through health promotion activities. Jock is passionate about providing Aboriginal and Torres Strait Islanders in our community with support and education to improve their health and wellbeing outcomes. 

Some ways Jock works with clients and the community are:

  • Promotion of SCHS services and programs at local events.
  • Attending home visits with
    SCHS staff
  • Cultural support for clients attending SCHS services or appointments
  • Supporting clients with referrals and filling out forms
  • Working collaboratively with partners and other service providers on projects

  • Weekly Cultural Walking Group
