Home & Community Care Program for Younger People

HACC-PYP provides services to support younger people with disabilities and their carers. These services help people live as independently as possible in the community.

Eligibility

  • Younger people from birth to age 65, or 50 years for Aboriginal and Torres Strait Islander peoples with moderate, severe or profound disabilities, and their unpaid carers’.
  • Eligibility means that the person is assessed as being in the HACC-PYP target group.
  • Eligible people are prioritised for service provision by the RN Community Care and Team Leader Aged care Services.
  • Eligibility for services is not based solely on age, but on the level of difficulty a person experiences in carrying out tasks of daily living.
  • There is no restriction on access to HACC-PYP services based on residency status or visa type.

Who is not eligble?

  • Age 65 and over, or 50 years and over for Aboriginal and Torres Strait Islander Peoples and their unpaid carers’.
  • Residents of NSW
  • Carers of NDIS participants

Cost

The fees for service are very reasonable and are worked out based on your income and are in line with the HACC-PYP Schedule of Fee’s Framework.

The fees for service are discussed with you, and set out in your service agreement for you at the time of your in-home assessment.

Service Information

The Home and Community Care Program for Younger People (HACC PYP) provides services to support younger people with disabilities, and their carers. These services help people live as independently as possible in the community.

Younger people are defined as people aged less than 65 years, or Aboriginal or Torres Strait Islander people aged less than 50 years.

You can get different types of support, depending on your particular needs.

The aim of HACC-PYP is to:

  • provide community care services, both in the home and in the community, to support younger people who have difficulty with the activities of daily living and/or their carers to remain as independent as possible;
  • provide a coordinated and integrated range of basic maintenance, social support and health services to younger people who have difficulty with the activities of daily living and/or their carers;
  • support eligible younger people to be more active and independent at home and in the community, thereby enhancing their quality of life and/or preventing premature admission to residential care;
  • provide flexible, innovative, contemporary, timely and responsive services.


Some people have more complex needs, which cannot be met by mainstream HACC PYP services alone. In these cases, HACC PYP services can point you to other programs that can help.
